\u2022 Professional Fee: see Table 1 above for \u2018Total Amount Paid\u2019
For Non-ODB recipients<\/strong>
When submitting a claim for an eligible person who does not have ODB coverage, pharmacists must
submit the following information:
\u2022 Patient Gender: \u2018F\u2019 = female; \u2018M\u2019 = male
\u2022 Patient Date of Birth: Valid YYYYMMDD
\u2022 Patient\u2019s Ontario Health Card number*
\u2022 Intervention codes:
▪ PS: Professional Care Services
▪ ML: Established eligibility coverage (i.e., 1 day of the Plan \u2018S\u2019 coverage)
\u2022 Carrier ID: \u2018S\u2019
